SOUTHLAKE, TX

CLASS SELECTION: When filling out the registration form, please indicate your preferred class time within your child’s age group for each participant. Each class meets once a week for 8 weeks on the same day and time. We kindly ask that you also provide an alternate class time, just in case your first choice is full, adjusted, or removed from the schedule. If any changes to your selected class are expected, we’ll be sure to keep you informed.

SIBLINGS: You can sign up all siblings on one form.

FRIEND REQUEST: If you are making friend requests, it is important to list the names in the note section of the form.

PAYMENT: We recommend you have your credit card ready as you will not be able to go back later to complete payment. Payment is required to submit the registration form in order to reserve a spot in a class and to confirm your t-shirt order. In the event that we are unable to place you in one of your desired classes, we will refund you in full.

The cost of registration covers:
- full 8 week season (8 total classes)
- new Hot Shots Soccer t-shirt
- end of season celebratory medal!

FACILITY RULES: All players must have an active player card to participate in the facility and a signed waiver. Player cards are $5/month or $50/year.
