Fill out a form for each participant you would like to sign up

After determining your class age and looking at the class schedule offered this Spring, determine your top 3 class preferences and indicate them on your form

It's important to list 3 preferences (if possible) because of the chance a class fills up, changes times or is removed from the schedule completely

Indicate all items you plan on purchasing: class, t-shirt and/or trophy

If you plan on purchasing a t-shirt or trophy, fill in the requested information to record your order

You can upload your waiver/release form through the registration page. Every participant must turn in a signed form prior to participation in class. You can submit it online, email it to me at , send in the mail, turn it in at our early registration event or at your first class.

Once successfully completed, your submission will be reviewed for class placement. Class assignments will be sent out 1 week before class starts.

Your assigned class will meet once a week on the same day and time for 8 weeks.

Classes start the week of March 16th.
T-Shirts
Pre-season Deadline - February 28th
Last Chance Deadline - March 20th
**Shirts ordered by the pre-season deadline will be available to pick up before class starts at the early registration event or you can pick it up at your first class! If you order a shirt after the pre-season deadline but before the last chance deadline, your shirt will be ready to pick up at class either the 3rd or 4th week of class**
Payment
We now have the option to pay online! Or you can pay at our early registration event, by mail or at your first class with cash or check made out to 'Hot Shots Soccer'. Please submit this form HERE when paying by cash or check. Please mail checks only to:
